Over time, the long-term effects of gravity, exposure to sun, dry air, and general stress takes its toll on an individual's face and neckline. A facelift can considerably reduce these signs of aging. Traditionally, plastic surgeons have performed facelifts that focus on tightening tissues, such as the skin and muscle, in order to decrease wrinkles and get rid of excess drooping skin. Dr. Youn recently coauthored a scientific article in the Aesthetic Surgery Journal to teach other plastic surgeons the most current thinking in facial aging: that aging in the face is due to loss of volume, in addition to droopy skin. Therefore, the 3-D Volumetric Facelift focuses on restoring volume to the face as well as removing excess sagging skin and fat from the face and neck. NO. These one hour or lunchtime type facelifts which you see advertised by many dubious cosmetic surgery centers are very different from the Volumetric Facelift. There are very few reputable plastic surgeons who perform these one hour facelifts because of their extremely limited results. These controversial procedures are performed in various clinics by physicians who are not necessarily plastic surgeons, often by young doctors trying to practice a new technique. They are basically marketing gimmicks in which non-reputable doctors cut corners to save time and try to operate on as many people per day as possible. Rest assured, the Volumetric Facelift is a state of the art surgery which has been published in the leading cosmetic plastic surgery journal.
There is usually very little discomfort with the Volumetric Facelift. Patients may take a few pain pills, although some patients don’t need any at all.
No. There are some patients who may not need or want volume replacement. For these select patients Dr. Youn often recommends a muscle-tightening facelift without fat transfer.

In selected patients, Dr. Youn performs a state-of-the-art traditional muscle-tightening facelift and necklift. This can often be used in patients who are considering a procedure with less swelling and downtime, in addition to select patients who have retained a considerable amount of volume in the face with aging. This procedure is compared to the facelifts of most outstanding plastic surgeons, with the exception that Dr. Youn utilizes a short, hidden scar and emphasizes a most natural look. Recovery time is minimal, and most patients don’t take more than a few pain pills.

Often termed the “weekend facelift,” submental liposuction removes the fat under the chin and in the neck to produce a sharper jawline and neckline. The fat may be resistant to exercise and diet. This procedure is best in patients who still have elastic skin and involves a small 1 cm incision under the chin. Often the results can be very dramatic. Patients who are older and/or have loose skin under their chin are not as good candidates for this procedure. Patients usually don’t need any pain pills, and often look great after just a couple days.

Facial aging involves both drooping of the skin and a loss of volume. While the loosening of the skin is usually most evident by the time you reach your forties or fifties, loss of volume can be seen as early as your late twenties. Loss of volume, or a gaunt look to the face, is most often seen in the lips, the cheeks, and under the eyes. This is best treated by Facial Fat Grafting or Transfer.
Dr. Youn is a protégé of Dr. Richard Ellenbogen, one of the pioneers of facial fat grafting. Dr. Youn recently coauthored a presentation with Dr. Ellenbogen on the latest techniques of facial fat grafting at the annual Aesthetic Surgery meeting, which received national attention by the New York Times and the E! Channel, among others. These techniques are a powerful tool in facial aging surgery, and can be used to improve the appearance of people of all ages. Fat is removed from certain areas of the body, purified, and then placed into certain areas of the face to regain facial volume that may be lost through aging, heredity, or other factors. The amount of fat that lasts permanently varies per patient and per area, but can be maximized using the advanced, state-of-the-art techniques that Dr. Youn employs. There is virtually no pain, no visible external scars, and little downtime with his techniques in facial fat grafting.
Dr. Youn’s technique for facial fat grafting is less traumatic than most traditional fat grafting techniques, yet still results in a high fat retention in most patients. Most patients look great within 1-2 weeks. Patients who smoke or have extremely high metabolism may have less fat remain than the average patient.
Fat grafting to the lips can be performed in the office with oral sedatives and local anesthesia for a more permanent lip filler.

Eyelid surgery is designed to remove excess skin and fat from the upper and lower eyelids.
An upper blepharoplasty is used to correct drooping upper eyelids that tend to sag with time. A thin scar is placed in the crease of the upper eyelid, where it is well-hidden. Excess skin is removed, often along with herniating fat. The procedure usually takes about 1 hour and can be performed using sedation or general anesthesia. Stitches can be removed after five days, and the discomfort is very minimal. Most patients don’t take any pain pills. Patients usually look good within 1-2 weeks, with most of the swelling gone by 1-2 months.
Insurance plans occasionally cover upper blepharoplasty surgery when the upper eyelid skin is so excessive that it disturbs the vision. Lower blepharoplasty is used to remove excess puffiness or bags from under the eyes. The lower eyelid puffiness is caused by excess herniating fat, which is removed during the procedure. In general, lower eyelid wrinkles are not removed by lower blepharoplasty. Wrinkles are best treated using lasers or chemical peels.
Lower blepharoplasty can be performed under sedation or general anesthesia, takes about an hour and fifteen minutes, and results in very little discomfort. Most patients don’t take pain pills and look good within a week or two.
Dr. Youn performs lower blepharoplasty using one of two techniques:
The traditional technique entails a small incision under the eyelashes and is best for patients who have excess skin of their lower eyelids in addition to puffiness. Skin can be removed, along with the herniating fat. The scar usually heals so well that it is extremely difficult to see.
The state-of-the-art, ‘scarless’, transconjunctival technique utilizes an incision inside the eyelid, allowing Dr. Youn to remove the lower eyelid fat without any visible external scars. This is a fantastic technique for younger people without loose skin or people who do not desire any visible scars. Often, chemical peels can be used to improve any fine wrinkling under the eyes. This can be done at the same time as the transconjunctival blepharoplasty.

A forehead lift or browlift is used to correct a drooping browline (eyebrow area) reducing the horizontal lines and furrows (worry lines) that often make a person appear angry, sad or tired. The procedure creates a refreshed, youthful appearance to the forehead area above the eyes.
When performing a forehead lift procedure, muscles and tissue areas that droop or cause creases are removed or corrected to smooth the skin on the forehead to reduce frown lines and raise the eyebrows. There are two surgical methods, and during your initial consultation the procedure that works best for you will be recommended.
Dr. Youn most often performs the Endotine Endoscopic Browlift. This enables the procedure to be performed with a minimal number of incisions and a minimum of downtime. The earliest endoscopic browlifts used permanent or removable titanium screws to keep the forehead skin elevated until it healed in place. With the revolutionary Endotine system, Dr. Youn is able to perform an endoscopic browlift using a small absorbable implant instead of metal screws. The brow is lifted, creating a refreshed and youthful look.
The traditional method of browlift utilizes a longer surgical incision that is hidden just above the hairline. Dr. Youn recently coauthored a paper in the Aesthetic Surgery Journal which described the technique he uses for this type of browlift. By utilizing this technique, the wrinkles are so well eradicated that patients often no longer need Botox treatments in the forehead afterwards. Dr. Youn most often uses this technique in patients who are not good candidates for the Endoscopic Browlift.

Surgery to reshape the nose is among the most common plastic surgery procedures. This procedure can reduce or increase the size of the nose, remove an unwanted hump, change the shape of the tip or the bridge, narrow the width of the nostrils, or change the angle between the nose and the upper lip. The procedure is also used to correct problems resulting from injury and can help relieve certain breathing difficulties. The majority of the rhinoplasties that Dr. Youn performs are done with a “closed technique” which limits any external scarring and incisions. Patients often have no visible scars after this surgery. He has trained under Dr. Richard Ellenbogen, who is recognized nationally as a specialist in correcting unsatisfactory surgeries done by other surgeons.

Chin augmentation can change the shape of the chin to extend the chin from the face. The contour of the chin can be reshaped to be smoother or more angular in appearance. Dr. Youn performs this procedure using either the patient’s own fat or a small implant placed through the mouth with no external scars. Chin augmentation can be tailored to create a more balanced yet feminine look in females, and a stronger, more masculine appearance in males. Swelling is usually minimal, and most patients can return to work within a few days.

Full and vivacious lips have always been recognized as aesthetically important features which signify youth and beauty. Some of us however, are born with thin, ill-defined lips, or lose the fullness in their lips as they grow older. Dr. Youn performs lip enhancement in a variety of ways, including using fat grafting or collagen to increase the fullness and enhance the shape of lips.

The corners of the mouth tend to droop as people age. Dr. Youn corrects this in two different ways. For minor drooping, injections of Restylane, Juvederm, or fat can provide a lift for the corners of the mouth. For patients with more severe drooping, a surgical corner mouthlift can be performed. It is a procedure which can be performed in the office under local anesthetic, and can result in a significant lift with a small scar.
